Supporting Young People Moving from Children’s Services into Adulthood

At Care NW, we understand that the transition from children’s services into adulthood can be one of the most important and challenging stages in a young person’s life. Many young people with complex needs experience uncertainty, placement instability, or gaps in support during this period due to the limited availability of specialist transition-focused services.

Our Specialist Transitional Services have been developed to bridge this gap by providing safe, structured, and person-centred residential and supported living pathways for young people aged 16–24 who require additional support as they move towards adulthood and greater independence.

A Structured Pathway Towards Independence

Our transitional model is designed around progression, stability, and long-term positive outcomes. We recognise that every young person develops at their own pace, which is why our support pathways are fully tailored to individual needs, risks, strengths, and aspirations.

Our services range from highly supported residential environments with intensive one-to-one support through to lower-dependency supported living arrangements focused on independence-building and community integration.

Our pathway approach includes:

Stabilisation

Providing safe, structured, and highly supported environments for young people who may be experiencing emotional distress, behavioural challenges, placement instability, or difficulties transitioning into adult services.

Development

Supporting young people to develop life skills, emotional regulation, confidence, communication skills, routines, and independence through consistent and person-centred support.

Progression

Helping individuals transition towards greater independence, community participation, education, employment opportunities, and lower levels of support where appropriate.

Person-Centred and Outcome-Focused Support

At Care NW, we place the individual at the centre of everything we do. We work closely with young people, families, social workers, commissioners, NHS professionals, key workers, and wider multidisciplinary teams to ensure support is coordinated, responsive, and aligned with each individual’s wider care and support plan.

Care NW benefits from a leadership team with significant experience across both CQC and Ofsted-regulated environments, including supporting young people with complex and high-dependency needs within children’s residential settings.
