Care NW provides flexible support pathways ranging from lower-level supported living through to residential care, one-to-one support, and specialist high-dependency support where required. Support may include assistance with mobility, transfers, personal care, medication management, nutrition, daily living activities, attending appointments, accessing the community, and maintaining physical wellbeing and independence.
We work closely with individuals, families, social workers, occupational therapists, physiotherapists, district nurses, commissioners, NHS professionals, and wider multidisciplinary teams to ensure support is coordinated, responsive, and aligned with each individual’s wider health and care plan.

We place strong emphasis on promoting independence wherever possible, supporting individuals to build confidence, maintain routines, engage with their communities, and participate fully in day-to-day life.
