Our experienced teams work closely with individuals to develop personalised support pathways focused on emotional wellbeing, communication, independence-building, and improving quality of life.
Support may include assistance with daily living skills, emotional regulation, communication support, sensory needs, education and employment opportunities, social interaction, community engagement, routines, budgeting, meal preparation, medication management, and developing confidence in day-to-day living.
We adopt proactive and person-centred approaches, including Positive Behaviour Support and structured care planning where appropriate, helping individuals manage anxiety, reduce distress, and develop coping strategies within supportive and predictable environments.
Care NW works closely with families, social workers, commissioners, NHS professionals, psychologists, occupational therapists, speech and language therapists, and wider multidisciplinary teams to ensure support is coordinated, responsive, and aligned with each individual’s wider care and support plan.
