We support individuals with a wide range of physical disabilities, mobility needs, long-term health conditions, and sensory impairments. Our services are designed to reduce barriers and maximise independence, whether through assistance with mobility, personal care, communication, accessing the community, daily living activities, or maintaining health and wellbeing.

Care NW provides flexible support pathways ranging from lower-level supported living through to residential care, one-to-one support, and more complex high-dependency care arrangements where required. Support is adapted around the individual and may include assistance with daily routines, medication management, attending appointments, education or employment opportunities, budgeting, meal preparation, and social or community engagement.

We work closely with individuals, families, social workers, occupational therapists, commissioners, NHS professionals, physiotherapists, and wider multidisciplinary teams to ensure support is coordinated, responsive, and aligned with each individual’s personal goals and wider care plan.

Our aim is not simply to provide care, but to empower individuals to build confidence, maintain independence, improve quality of life, and participate as fully as possible within their communities.
